If you have not already done so, submit the
following medical records for your elementary student(s):
copy of complete
and updated immunization form or religious exemption form
copy of school entrance medical examination report
copy
of birth certificate and social security card
CURRICULUM
LIST
At
the beginning of the school year, submit a Curriculum List,
recording the subjects your elementary student is covering, the grade
level, and the source or publisher.
QUARTERLY REPORTS
Parents/teachers of elementary students
should send in a Quarterly Report and Attendance Record
every three months.
Grading Scales:
Elementary Grades 1-3 may
use letter grades or test grade percentage
Elementary Grades 4-5 should use test grade
percentage for all tested subjects and letter grades
for untested material
